Lower Blepharoplasty in Pune: Targeting Under-Eye Bags

Lower blepharoplasty is a surgical procedure that addresses under-eye bags, puffiness, and lower eyelid skin laxity. For patients bothered by persistent under-eye bags that no skincare routine or lifestyle change has been able to resolve, it is one of the most effective and durable solutions available. This guide covers why under-eye bags form, who lower blepharoplasty is appropriate for, the two main surgical approaches, what recovery involves, and how to decide if this is the right step for you.

Lower blepharoplasty removes or repositions the fat pockets causing under-eye bags and, where needed, addresses excess skin beneath the eye. The transconjunctival approach (no visible scar) is used for fat-only concerns. The subciliary approach (fine incision below the lashes) is used when skin laxity is also present. Recovery is typically 10 to 14 days. Cost in Pune ranges from Rs. 45,000 to Rs. 70,000.

Why Under-Eye Bags Form?

Understanding the cause of under-eye bags helps explain why lower blepharoplasty is effective when other approaches are not.

The fat around the eyeball is contained within a compartment by a thin membrane called the orbital septum. With age, this membrane weakens and stretches, allowing the orbital fat to herniate forward. This produces the characteristic rounded puffiness below the eye that is described as under-eye bags.

This process is largely genetic. Some people develop it in their thirties, others not until their fifties or later. Lifestyle factors such as salt intake, fluid retention, and sleep quality affect the degree to which bags appear day to day but do not cause or cure the underlying structural change. Once the orbital septum has weakened and fat has herniated forward, it will not reverse without surgical intervention.

This is why eye creams, cold compresses, and under-eye fillers can reduce the appearance of bags temporarily but cannot correct the underlying fat herniation. Surgery is the only reliable permanent solution.

When Is Lower Blepharoplasty the Right Choice?

Lower blepharoplasty is the appropriate procedure when the primary cause of the under-eye appearance is fat herniation, with or without associated skin laxity. It is not indicated for every under-eye concern.

The procedure is appropriate when:

  • Persistent under-eye bags are present that have not improved with lifestyle measures
  • The bags are caused by fat herniation (identifiable by their rounded, soft quality and persistence regardless of rest or hydration)
  • The patient is in good general health
  • Dry eye syndrome, if present, has been assessed and is not severe enough to contraindicate surgery
  • Expectations are realistic regarding the degree of improvement and the recovery involved

Lower blepharoplasty is less appropriate when the primary concern is a tear trough depression (the hollow groove beneath the bag, rather than the bag itself), skin discolouration, or orbital rim hollowing. These concerns are better addressed through facial fat grafting or under-eye dark circle treatment. In some patients, surgery and fat grafting are performed together.

The Two Surgical Approaches to Lower Blepharoplasty

The choice of surgical approach is one of the most important decisions made during consultation and is determined by the patient’s specific anatomy.

Transconjunctival Lower Blepharoplasty (No External Scar)

The transconjunctival approach places the incision entirely inside the lower eyelid, on the conjunctival surface. There is no external incision and no visible scar on the skin.

Through this incision, the herniated orbital fat is accessed and either removed or repositioned. Fat repositioning is often preferred over pure excision because it avoids the hollowed appearance that can result from aggressive fat removal. Repositioned fat is spread along the orbital rim to soften the transition between the lower eyelid and the cheek.

This approach is the right choice when:

  • The skin quality of the lower eyelid is good, with no significant excess or wrinkling
  • The primary concern is fat herniation (bags) with minimal skin laxity
  • The patient wants to avoid any external scar

Recovery from the transconjunctival approach is generally shorter and involves less bruising than the subciliary method.

Subciliary Lower Blepharoplasty (With External Incision)

The subciliary approach uses a fine incision placed just below the lower lash line. This incision heals well and, in most patients, is not visible once fully healed. It is used when there is meaningful skin excess in the lower eyelid that would not be adequately addressed through the transconjunctival approach alone.

Through the subciliary incision, the surgeon accesses both the fat and the skin, allowing for precise control over both components. A strip of excess lower eyelid skin is removed along with the fat correction.

This approach requires careful technique to avoid two specific complications: ectropion (lower eyelid turning outward, caused by over-removal of skin or excessive tension on the lid) and scleral show (an increase in the visible white below the iris, caused by lower lid retraction). Both are uncommon when the procedure is performed by an experienced surgeon with appropriate patient selection.

Fat Removal vs Fat Repositioning: Which Is Better?

This is a question patients frequently ask, and the answer has shifted in surgical thinking over the past decade.

Historically, lower blepharoplasty involved removing herniated fat. The concern with fat removal alone is that it can produce a skeletonised or hollow appearance beneath the eye, which replaces one problem (bags) with another (hollowness and shadow).

Fat repositioning, in which the herniated fat is released from its herniated position and spread along the orbital rim rather than excised, produces a fuller and more natural result by softening the transition between the lower lid and the cheek. It is the preferred technique in most cases at Dr. DC Plastic Surgery in Pune.

In some patients, a combination of partial excision (removing the small amount of truly excess fat) and repositioning (redistributing the remainder) is the most appropriate approach. The surgical plan is determined by the degree of herniation and the anatomy of the tear trough.

The Consultation at Dr. DC Plastic Surgery

A lower blepharoplasty consultation with Dr. Dhruv Chavan involves:

  • Assessment of lower eyelid skin quality and quantity
  • Evaluation of orbital fat distribution (the three compartments of lower eyelid fat are assessed individually)
  • Assessment of the tear trough and orbital rim anatomy to determine whether fat repositioning will improve or worsen the transition zone
  • Evaluation of lower lid tone and snap-back (to assess the risk of ectropion from skin excision)
  • Dry eye screening
  • Discussion of surgical approach (transconjunctival vs subciliary) and reasoning
  • Realistic expectations for recovery and results

Consultations are available at Dr. DC Plastic Surgery’s multiple Pune clinic locations.

Recovery from Lower Blepharoplasty

Transconjunctival Approach Recovery

  • Days 1 to 3: Mild swelling, some bruising under the eye. Cold compresses help. Eye drops prescribed for comfort.
  • Days 3 to 7: Bruising and swelling begin to reduce. Most patients are comfortable in social settings by the end of the first week.
  • Weeks 1 to 2: Return to desk-based work for most patients.
  • Months 1 to 3: Final results become fully apparent as residual swelling resolves.

Subciliary Approach Recovery

  • Days 1 to 5: More pronounced bruising and swelling. The lower eyelid will look tighter than the final result.
  • Days 5 to 7: Sutures removed.
  • Weeks 2 to 3: Visible bruising largely resolved. Most patients return to work.
  • Months 2 to 4: Swelling fully resolves. The incision line fades to an almost invisible fine line below the lashes.

For both approaches, screen use should be limited in the first week as dry eyes are common in the early recovery period. Lubricating eye drops are typically prescribed.

Risks of Lower Blepharoplasty

  • Dry eyes: Temporary worsening of tear film stability is common. Patients with pre-existing dry eye require careful evaluation.
  • Ectropion: Lower lid turning outward. Risk is higher with the subciliary approach and is mitigated by conservative skin excision and adequate lid tone assessment before surgery.
  • Scleral show: Increased visibility of the white below the iris, caused by mild lower lid retraction. Usually temporary.
  • Asymmetry: Difference between the two lower eyelids in the degree of correction.
  • Hollowing: Over-removal of orbital fat. Avoided by conservative excision and repositioning.
  • Infection or haematoma: Uncommon but possible with any surgical procedure.

Frequently Asked Questions

Will the bags ever come back after surgery?

Fat herniation, once corrected, does not typically recur. However, ageing continues and further eyelid changes can develop over time. The improvement relative to the pre-operative baseline is long-lasting, usually more than a decade.

Can under-eye fillers do the same job as surgery?

Fillers can reduce the appearance of bags by adding volume to the tear trough area, blending the shadow between the bag and the cheek. However, they do not address the fat herniation itself. For patients with significant bags, fillers often worsen the appearance by adding volume around an already prominent fat pad. For patients with mild bags and a prominent tear trough, fillers may be sufficient. Your surgeon will advise during consultation.

Is the transconjunctival approach always possible?

No. Patients with significant skin laxity in the lower eyelid are better served by the subciliary approach because the transconjunctival route cannot address excess skin.

Can I have lower blepharoplasty without upper blepharoplasty?

Yes. The two eyelid areas are independent. However, many patients find that addressing both produces a more balanced result.
