Where Are Implants Placed in the Breast?

Implant placement is determined by each patient’s tissue thickness, skin quality, chest muscle anatomy, degree of sagging, desired size, and activity level. Dr. Dhruv Chavan uses two placement approaches.

Dual plane is the most commonly performed placement at the clinic. The implant sits partially beneath the chest muscle in the upper portion, adding an extra layer of muscle coverage that hides implant edges and upper borders. This produces a fuller, more natural upper pole contour and gives the appearance of natural augmentation even with a relatively smaller implant size. Dual plane placement is especially beneficial for thinner patients with limited soft tissue coverage, and consistently delivers the most natural-looking, long-lasting results.

Subfascial placement positions the implant below the muscle fascia but above the muscle itself. This approach is preferred in patients with adequate soft tissue coverage and minimal sagging, offering good implant support with a faster recovery. Neither subglandular (above the muscle) nor full submuscular placement is routinely offered.

What Incision Technique Is Used?

The inframammary fold incision (under the breast crease) is Dr. Dhruv Chavan’s preferred approach for the majority of cases. This ~3-4 cm incision provides excellent surgical visibility and precision for implant placement, and the resulting scar is well hidden within the natural breast crease.

All implants are inserted using a Keller funnel, a sterile no-touch technique that allows the implant to be placed without contact with skin or gloves, reducing contamination risk and helping prevent capsular contracture. Periareolar incision (around the nipple) may be used selectively depending on areola size, breast anatomy, and patient preference.
