The First 24 Hours After Rhinoplasty

The first 24 hours after rhinoplasty are spent in post-operative recovery at the surgical facility before discharge. Expect swelling and bruising around the eyes and nose, a nasal splint in place, and some nasal congestion from internal swelling and any internal dressings. Light bleeding from the nostrils in the first 24 hours is normal and managed with a drip pad placed under the nose.

Key instructions for the first 24 hours:

    • Head elevation: Keep the head elevated at 30 to 45 degrees at all times, including during sleep. Sleeping flat increases blood flow to the face and worsens swelling significantly. Use extra pillows or a recliner chair.
    • No blowing the nose: Any pressure inside the nasal cavity risks displacing structures that are not yet healed. Gently dab the nostrils with a clean tissue if needed.
    • Cold compresses on the cheeks (not on the nose): Gently applied cold on the cheeks and around the eye area reduces swelling. Do not place anything directly on the nose itself.
    • Take prescribed medications as directed: Pain relief, any antibiotics prescribed, and anti-nausea medication if needed. Do not self-supplement with aspirin, ibuprofen, or any blood-thinning supplement without instruction from your surgeon.
    • Rest, no screens for prolonged periods: Eye strain does not affect the rhinoplasty directly, but fatigue impairs healing and rest is the most productive use of the first 24 hours.

Week 1: Splint On, Swelling Peaks

The nasal splint remains in place for 7 to 10 days after rhinoplasty. During this time the nasal bones are consolidating in their new position, held by the splint externally. The splint is not uncomfortable but it is important: removing it early or allowing it to get wet can affect the result.

What is normal in Week 1:

Significant bruising around both eyes (black eyes are normal and expected, particularly after rhinoplasty involving osteotomies). Swelling that peaks at 48 to 72 hours and then begins to reduce gradually. Nasal congestion from internal swelling, which makes breathing through the nose feel blocked. Numbness of the nasal tip, particularly in open rhinoplasty, from temporary disruption of fine nerve branches. Some tenderness and sensitivity around the nose.

What to do:

Maintain head elevation during sleep throughout Week 1. Continue the same general care guidelines from Day 1. Eat soft foods that do not require wide mouth opening or facial tension. Keep all follow-up appointments. Splint removal is typically at the 7 to 10 day mark.

What to avoid:

Strenuous activity of any kind. Bending forward. Lifting anything over a few kilograms. Any pressure or impact on the nose. Getting the splint wet (cover the face carefully during showering; use a damp cloth for the face rather than showering the face directly).

Week 2: Splint Removed, Visible Improvement Begins

Splint removal at 7 to 10 days is a significant milestone. The nose is still swollen and will not look like the final result, but the shape changes become visible for the first time. Bruising around the eyes fades substantially through Week 2, from dark purple to yellow-green to near-normal skin colour by the end of the week in most patients.

“Patients are often surprised that the nose still looks swollen and wide at two weeks,” says Dr. Dhruv Chavan, plastic surgeon at Dr. DC Plastic Surgery, Pune. “I explain this at every consultation: the shape at two weeks is the swollen version of the result. The tip in particular holds swelling for many months. What matters at two weeks is that the structural changes are in place and healing correctly. The final appearance takes patience.”

What changes after splint removal:

Social presentability improves considerably. Many patients with desk-based work return to the office at 10 to 14 days, when bruising has faded enough. Light activity such as walking is permitted from around Week 2 onward.

What to continue avoiding:

Contact sports, swimming, and any activity that risks impact to the nose. Heavy lifting and exertion that raises blood pressure. Glasses resting on the nasal bridge (see below for glasses guidance). Significant facial expressions that put tension on the nose (laughing hard, wide smiling) are fine but worth being gentle about in the first two weeks.

Weeks 3 to 6: The Bones Consolidate

This is the most important period for protecting the nasal structure. After osteotomies (bone cuts) to narrow the nose or correct the nasal bones, the bones are healing and consolidating in their new position. They are stable enough for normal life but not yet ready for impact.

Key restrictions in Weeks 3 to 6:

No contact sports: football, basketball, cricket, boxing, any sport where impact to the face is possible. No swimming in pools (chlorine) or open water. No heavy weight training or high-intensity cardio that significantly raises blood pressure. Avoid sleeping on the side or face-down; back-sleeping with head elevated remains the recommended position through at least Week 6.

The glasses rule:

Glasses resting on the nasal bridge must not be worn for the first 6 weeks after rhinoplasty in patients who had osteotomies. The weight and pressure of a glasses frame on the nasal bones can depress the nasal dorsum while the bones are still consolidating. Use contact lenses, tape glasses to the forehead, or use glasses taped to avoid contact with the bridge for this period. “The glasses restriction is the one patients find hardest to follow, particularly those who wear glasses all day for work,” says Dr. Dhruv Chavan, rhinoplasty surgeon at Dr. DC Plastic Surgery. “Taping the glasses to the forehead looks unusual but it works. I would rather explain the tape than explain why the dorsum has a depression from glasses pressure. The 6-week mark is not a guess: it reflects when the bone union is reliable enough to tolerate contact.”

Sun Exposure and the Nasal Scar (Open Rhinoplasty)

For open rhinoplasty, there is a small transcolumella scar across the strip of tissue between the nostrils. This scar heals well and becomes very inconspicuous in most patients because the columella is not a sun-exposed area in daily life.

Sun exposure to a healing scar before it has fully matured (typically 6 to 12 months) can cause permanent hyperpigmentation: the scar turns darker than the surrounding skin and becomes more visible rather than less. For the first 12 months after open rhinoplasty, apply SPF 30 or higher to the nose whenever you are outdoors in direct sun.

Indian skin phototypes (Fitzpatrick III to V) are at higher risk of post-inflammatory hyperpigmentation in healing scars than lighter skin types. Consistent sun protection throughout the first year of healing is more important for Indian patients than for those with lighter skin.

Weeks 6 to 12: Returning to Normal Activity

By 6 weeks, the nasal bones are reliably united and most activity restrictions lift. Gym training, swimming, cycling, and most sports can resume. Contact sports and combat sports should remain off until 3 months at minimum and ideally longer.

The nose at 6 to 8 weeks looks substantially better than at 2 weeks, but swelling is still present, particularly at the nasal tip. The tip softens and refines over months, not weeks. Patients with thicker nasal skin see slower swelling resolution than those with thinner skin; tip refinement may not be complete until 12 to 18 months.

This is the normal trajectory, not a sign that anything is wrong. The surgical changes were made at the time of the operation. The healing process reveals them progressively.

What Is Normal vs What Requires Attention?

Normal after rhinoplasty:

Swelling and bruising for 2 to 3 weeks. Nasal congestion that improves gradually over 3 to 4 weeks. Numbness of the nasal tip for several months, particularly after open rhinoplasty. Asymmetry in the early weeks as different areas swell and resolve at different rates. Temporary worsening in appearance at around Week 3 to 4 when internal swelling redistributes.

Contact your surgeon if you notice:

Increasing pain rather than gradually improving pain beyond 48 to 72 hours. Fever above 38 degrees Celsius. Fresh red bleeding (not old dried blood). Significant asymmetry that worsens rather than gradually resolves. Any unusual discharge or odour from the nose. Any change in the position of the splint before it is due for removal.

When can I go back to work after rhinoplasty in Pune?

Most patients with desk-based or non-physical work return to the office between 10 and 14 days after rhinoplasty, when the splint has been removed and bruising has faded enough to feel socially comfortable. Patients in client-facing roles may prefer to wait until 2 to 3 weeks when residual bruising is minimal. Physical work or roles requiring significant exertion should be deferred for at least 4 to 6 weeks. Gentle nose blowing is generally permitted from around 3 to 4 weeks after rhinoplasty, once internal healing has progressed sufficiently. Do not blow your nose forcefully before this. If you have significant congestion, saline nasal rinses or nasal sprays recommended by your surgeon can help. Avoid sneezing with a closed mouth in the first several weeks: if you need to sneeze, let it out through an open mouth to avoid pressure build-up in the nasal cavity.

Is it normal for my nose to look worse at some point during recovery?

Yes. Swelling does not reduce uniformly. Around Weeks 3 to 5, many patients notice what feels like a worsening: the tip looks bulbous, the dorsum appears wider, or asymmetry seems more pronounced. This is normal swelling redistribution, not a sign of a problem. The structural work done at surgery is not undoing itself; swelling is temporarily obscuring the result. The improvement at Week 8 to 12 is typically dramatic compared to this mid-recovery phase.

When will I see the final rhinoplasty result?

The result is visible and pleasing well before it is final. Most patients are happy with the direction of the result from around 3 months onward, when the majority of swelling has resolved. The complete result, including full tip refinement and resolution of all residual swelling, takes 12 to 18 months and longer in patients with thicker nasal skin.
