How Breast Size Is Determined?
Before looking at causes of growth, it helps to understand what breast tissue is made of.
Breasts consist of two main components: glandular tissue (the milk-producing structures) and adipose tissue (fat). The balance of these varies significantly from person to person and is largely genetic. Hormones primarily oestrogen and progesterone regulate glandular tissue, while body composition determines the fat component.
When breasts grow larger, it is usually due to one or both of these components increasing in volume. Understanding which one is driving the change helps explain the cause.
Common Reasons Breasts Get Bigger
1. Hormonal Changes
Hormones are the single most common driver of changes in breast size throughout a woman’s life. Oestrogen and progesterone directly stimulate breast tissue, causing it to swell, become denser, or grow.
Menstrual cycle: The most cyclical cause. In the days before menstruation, progesterone levels rise and cause the breast ducts and glands to swell with fluid. Many women notice their breasts feel larger, heavier, and tender in the week before their period and return to their usual size shortly after it begins. This is entirely normal.
Puberty: The first major hormonal wave of breast development, typically beginning between ages 8 and 13. Growth can be uneven between sides and may continue into the late teens.
Perimenopause and menopause: As oestrogen levels fluctuate and then decline, breast tissue composition changes. Some women experience breast growth during perimenopause due to hormonal fluctuations, even as overall oestrogen eventually falls. Others notice a change in breast density and shape rather than size.
2. Pregnancy and Breastfeeding
Breast enlargement during pregnancy is expected and among the earliest physical signs for many women. From the first trimester, rising levels of oestrogen and progesterone stimulate glandular tissue to prepare for milk production. Breasts may grow by one or several cup sizes throughout pregnancy.
After delivery, prolactin the milk-production hormone keeps breast tissue engorged, particularly in the early weeks of breastfeeding. Breasts typically reduce in size once breastfeeding stops and hormone levels normalise, though they may not return to exactly their pre-pregnancy size or shape.
Some women find that after pregnancy and breastfeeding, their breasts are larger, sagging, or have changed in unpredictable ways. Breast lift or breast reduction surgery can address these post-pregnancy changes when lifestyle approaches are insufficient.
3. Weight Gain
Because breasts contain a significant proportion of fatty tissue, overall weight gain leads to breast enlargement in most women. The extent varies depending on individual fat distribution some women store proportionally more fat in their breasts, others less.
This works in both directions: weight loss typically reduces breast size, though the degree of reduction depends on the ratio of fatty to glandular tissue in your particular breasts.
If weight gain is the primary driver of breast enlargement and you are at a healthy weight for your height, there may not be any intervention needed beyond maintaining stable body composition. 4. Hormonal Contraceptives
Certain hormonal contraceptives particularly combined oral contraceptive pills and hormonal IUDs containing progesterone can cause breast enlargement as a side effect. Exogenous hormones can stimulate breast glandular tissue, similar to natural hormonal fluctuations.
The degree of enlargement varies by individual and by the specific contraceptive formulation. For many women it is mild and temporary, settling after the first few months of use. For others it is more pronounced and persists.

5. Hormone Replacement Therapy (HRT)

If breast enlargement on HRT is significant or causing discomfort, a review of the formulation or dosage with your prescribing doctor is the appropriate first step.
6. Thyroid Disorders
An underactive thyroid (hypothyroidism) slows the body’s metabolism and can lead to generalised weight gain including in the breasts. Women with undiagnosed or poorly controlled hypothyroidism often notice gradual weight gain across the body, of which breast enlargement may be one element.
If breast growth is accompanied by other symptoms such as fatigue, cold intolerance, dry skin, hair thinning, or unexplained weight gain, it is worth asking your doctor for a thyroid function test. Treating the underlying thyroid condition can lead to weight normalisation.
7. Medications
Several commonly prescribed medications can cause breast enlargement as a side effect:
- Antidepressants and antipsychotics some SSRIs, tricyclics, and antipsychotic medications increase prolactin levels, which can stimulate breast tissue
- Certain blood pressure medications including some calcium channel blockers
- Digoxin a heart medication with mild oestrogenic activity
- Some epilepsy medications including phenytoin

8. Macromastia (Clinically Large Breasts)
For some women, breast growth is not the result of a single identifiable cause it is simply that their breasts have grown substantially larger than average, often from puberty onward. This condition is called macromastia (or gigantomastia in severe cases), and it can be driven by a combination of genetic predisposition and heightened sensitivity of breast tissue to normal hormone levels.
Macromastia frequently causes significant physical symptoms: chronic neck, shoulder, and back pain; shoulder grooving from bra straps; skin rashes under the breast fold; restricted ability to exercise; and considerable emotional distress.
For women with macromastia, natural methods and lifestyle changes rarely provide adequate relief. Breast reduction surgery is the most effective intervention and one of the highest patient-satisfaction procedures in plastic surgery because of the immediate and lasting relief it provides.
9. Benign Breast Conditions
Some women notice localised breast enlargement or asymmetric growth caused by benign (non-cancerous) breast conditions:

- Breast cysts fluid-filled sacs that can enlarge and cause localised swelling, often varying with the menstrual cycle
- Lipomas benign fatty lumps that can develop within breast tissue
These conditions require diagnosis by a Plastic Surgeon or breast specialist, typically via ultrasound or mammography. Most are benign and require no treatment beyond monitoring, though some may be aspirated or surgically removed.
10. Breast Cancer (Rare but Important to Rule Out)
While the vast majority of breast growth is benign and hormonally driven, unexplained rapid growth, a new lump, skin changes, nipple discharge, or significant asymmetry that develops quickly should always be assessed by a Plastic Surgeon.
Breast cancer can, in rare cases, present as breast enlargement particularly inflammatory breast cancer, which can cause rapid swelling, redness, and warmth in the breast. If you notice any of the following, seek medical attention promptly rather than attributing it to hormonal changes:
- A new lump or thickening in the breast or armpit
- Skin that looks dimpled, puckered, or like orange peel
- Nipple inversion that is new
- Nipple discharge, especially if bloody or from one side only
- Rapid enlargement of one breast without an obvious cause
This is not meant to cause alarm these symptoms are far more likely to have benign explanations but they deserve prompt evaluation.

For many women, the causes above are temporary and self-resolving. Menstrual swelling passes. Post-pregnancy breast changes settle. Contraceptive-related growth stabilises.
But for others, breast growth is persistent, significant, and comes with real consequences:
- Physical symptoms chronic back, neck, and shoulder pain; shoulder grooving from bra straps; skin infections under the breast fold; restricted movement and difficulty exercising
- Postural problems forward rounding of the shoulders, spinal strain
- Emotional impact self-consciousness, difficulty finding clothing, social avoidance
- Functional limitations difficulty with physical activity, sport, or work
