Is Gynecomastia Different in Overweight Men
Overweight men can present with two distinct conditions, and distinguishing between them determines the surgical approach.
True gynecomastia is the presence of glandular breast tissue beneath the nipple-areolar complex. This tissue is firm and does not reduce with weight loss. It is hormonally driven and requires surgical removal regardless of the patient’s weight.
Pseudogynecomastia (also called lipomastia) is chest enlargement caused entirely by fatty tissue, without a glandular component. This is more common in overweight men and responds to liposuction without gland excision. Many overweight patients present with a combination of both. Does Weight Affect Candidacy?

Will losing weight before surgery improve results?
In some cases, yes. If skin laxity is a concern, achieving a more stable weight before surgery can reduce the need for additional skin reduction procedures.

Is liposuction alone sufficient for overweight men?
Liposuction alone is sufficient when the chest enlargement is caused entirely by fatty tissue and no firm glandular disc is present.
In patients with both glandular and fatty tissue, a presentation that is common in overweight men, a combined approach using liposuction followed by gland excision produces the most complete result. The appropriate technique is confirmed at examination.
Does higher BMI increase surgical risk?
Higher BMI can increase anaesthesia-related risk and may affect wound healing. These factors are assessed at the pre-operative consultation.
Patients with well-controlled weight and no significant comorbidities generally proceed without increased risk. Patients where BMI-related factors are a concern are counselled on optimising health before surgery is scheduled.
Does the chest stay flat after surgery if weight increases?
Liposuction results can be partially affected by significant weight gain after surgery, as remaining fat cells in the treated area can enlarge.
Glandular tissue that has been surgically excised does not return with weight gain. The risk of recurrence for the glandular component is low, provided the underlying hormonal cause was resolved before surgery. Maintaining a stable weight after the procedure supports the longevity of results.
