    The truth is, even if one side looks normal now, there is usually some underlying glandular tissue on both sides. In gynecomastia, the gland is often present bilaterally, just that it may be more visible on one side first.

    If surgery is done on only one side, the untreated side may enlarge later due to natural gland growth, body changes like fat deposition or future hormonal fluctuations. This can lead to an uneven, asymmetrical chest and the need for another surgery.

    By treating both sides together, your surgeon ensures that your chest looks balanced and symmetrical right from the start and stays that way in the long term. So any changes in the future happen equally on both sides and you dont end up with the same problem, just on the other side.

    I understand your concern. Many people with puffy nipples don’t feel any pain or hard lump, and that often creates confusion. But let me assure you — pain or a distinct lump is not required to diagnose puffy nipples.

    The puffiness itself happens because of glandular tissue beneath the areola. Even if you don’t feel it as a lump, it is still there, pushing out when the skin is relaxed.

    Medicines or exercise cannot treat this, because the problem is structural (extra gland tissue) and not hormonal in most adults.
    Workouts or dieting may reduce fat, but the gland will still remain and keep causing puffiness.
    The only effective and permanent solution is surgery — where the excess gland is carefully excised (often with liposuction to shape the chest). This flattens the nipple, restores a natural chest contour, and the result stays permanent.

    The good news is, it’s a minor day-care procedure with a very high success rate. Many patients feel a huge boost in confidence once the puffiness is gone.

    "I completely understand your concern — puffy nipples can be very frustrating, especially when they look flat for a while but then puff out again. This happens because of excess glandular tissue beneath the areola.

    When the skin is tight (like in cold weather or when pressed), the gland gets pushed down and the nipple looks flat. But once the skin relaxes, the gland pops out again — that’s why the puffiness always comes back.

    Now, regarding treatment:
    •Exercise and diet: These can help reduce fat and improve chest shape, but they cannot remove gland tissue. That’s why even lean or muscular men often still have puffy nipples.
    •Medicines: For most adults, this condition is structural, not hormonal. So no medicine can shrink or dissolve the gland.
    •Surgery: The only permanent and effective solution is surgery. Through a small incision at the edge of the areola, the gland is carefully removed (often combined with liposuction). Once the gland is excised, the nipple flattens and stays normal permanently.

    The goal is a natural, masculine chest contour and freedom from the constant worry of puffiness. Many of my patients say this gives them back their confidence — whether it’s wearing a T-shirt, going to the gym, or just feeling comfortable in their own body."

    The main reason is usually excess glandular tissue under the areola (a mild form of gynecomastia).
    • Fat alone does not cause the nipple to puff out. Even lean, fit men can have puffy nipples.
    • Dark skin (areola color) is completely normal and just depends on your natural skin tone — it is not a sign of any medical problem.
    So, if you see puffiness, it’s most likely because of the gland. Exercise and diet won’t remove it, and medicines don’t help in adults.

    In gynecomastia cases up to Grade 3, we use ultrasonic lipolysis along with nipple repositioning straps and proper strapping, which helps the skin tighten naturally and prevents sagging.

    However, if the case is very severe and there is excess skin, sometimes skin removal may be necessary. But we usually wait for 6–8 months to allow natural skin contraction. If the skin does not shrink by then, only in the second stage do we plan for skin removal.

    Our goal is to avoid unnecessary scarring.
