What you do in the weeks after blepharoplasty affects how the healing progresses, how quickly the swelling resolves, and how naturally the result settles. Patients who understand what to expect at each stage of recovery manage it better, worry less about normal changes, and identify the things that actually require attention. This guide covers blepharoplasty recovery in Pune from the first hours after surgery through the point where the result is stable and complete.
Blepharoplasty recovery in Pune takes 7 to 10 days before most patients feel socially presentable and return to desk work. Most swelling resolves within 3 to 6 weeks, with the final result visible at 3 to 6 months. Immediately After Blepharoplasty: The First Hours
Blepharoplasty is typically performed under local anaesthesia with sedation, or in some cases general anaesthesia, and is a day procedure for most patients. You will be monitored in the recovery area for 6 to 8 hours before discharge. You will need someone to accompany you and drive you home: you will not be in a condition to drive yourself.
What to expect immediately after surgery:
- Eyelids will feel tight and swollen. The periorbital area swells rapidly after eyelid surgery and the lids may feel heavier than normal. This is expected and peaks in the first 24 to 48 hours.
- Vision may be temporarily blurred. Lubricating eye ointment applied during the procedure, combined with early swelling, blurs vision transiently. This resolves as the ointment clears and early swelling settles.
- Some bruising will be visible immediately. The degree depends on individual factors, but most patients have some periorbital bruising from the first hours.
- Sensitivity to light. The eyes may feel light-sensitive for the first day or two. Resting in a darkened room or using sunglasses when light is uncomfortable helps.
Day 1 to 3: Managing Swelling and Discomfort
The first 72 hours are when swelling and bruising are at their most significant. This is the phase that requires the most care and also the phase that looks worst.
The most important step in this phase is head elevation. Keeping the head elevated at 30 to 45 degrees at all times, including during sleep, reduces blood pooling in the periorbital tissues and limits swelling significantly. Use extra pillows or a recliner chair. Sleeping flat will cause noticeably more swelling.
Cold compresses gently applied to the closed eyelids reduce swelling and discomfort in the first 48 hours. Use a clean cold pack or chilled gel eye mask; ensure it is not so cold as to cause skin discomfort, and apply for no more than 10 to 15 minutes at a time with breaks. From Day 3 onward, cold compresses are less effective and can be discontinued.
Do not rub or press on the eyelids. The tissues are in early healing and the incisions are fresh. Any pressure or friction on the lids should be avoided. Clean around the eyes gently with a clean damp cloth or as directed by your surgeon.
Take prescribed medications as directed. Pain relief is typically mild in blepharoplasty; discomfort is usually described as soreness and tightness rather than pain. Any antibiotics prescribed should be completed. Do not take aspirin, ibuprofen, or blood-thinning supplements unless specifically approved by your surgeon, as these increase bruising.

Week 1: Suture Removal and the First Review
Upper blepharoplasty sutures are typically removed at 5 to 7 days. For transconjunctival lower blepharoplasty (incision inside the lower lid), there are no external sutures to remove. For transcutaneous lower blepharoplasty (sub-lash incision), sutures are similarly removed at around 5 to 7 days.
“The suture removal appointment is also the first opportunity to assess how the healing is progressing,” says Dr. Dhruv Chavan, plastic surgeon at Dr. DC Plastic Surgery, Pune. “I look at the lid position, the symmetry of early healing, the incision quality, and whether the patient has any concerns. Most are relieved to see that the dramatic swelling of the first 3 days has already started to settle by this point.”
By the end of Week 1, bruising has typically begun its progression from dark purple or red-brown to a yellowish-green as the blood pigment breaks down. Swelling remains significant but is reducing. Most patients with desk-based work choose to return to the office at some point between Day 10 and Day 14, when residual bruising is minor enough to be covered with concealer if desired.
What to continue avoiding in Week 1:
Strenuous physical activity. Bending forward. Lifting heavy objects. Rubbing the eyes. Wearing contact lenses until the surgeon confirms the incision has healed adequately (typically 7 to 10 days minimum). Wearing eye makeup until healing is confirmed at the review.

Week 2: Returning to Normal Life
By Week 2, the dramatic appearance of the first days has passed. Most of the bruising has resolved or faded to a level easily managed with cosmetic cover. Swelling continues to reduce, though the eyelids will still feel slightly thicker and heavier than they eventually will.
Most patients are socially comfortable at 10 to 14 days. Returning to desk work, light social activity, and driving is appropriate for most patients in Week 2, assuming vision is clear and comfortable.
Eye makeup can typically be resumed from around 10 to 14 days for upper blepharoplasty, once the incision is healed and the surgeon has confirmed this at review. Eyeliner and mascara near the incision should be applied and removed gently; avoid anything that requires rubbing or significant pressure on the lids.
Contact lenses can typically be resumed at 10 to 14 days for upper blepharoplasty. For lower blepharoplasty, confirm with your surgeon at review, as the lens insertion and removal process involves gentle pressure on the lower lid.
Light exercise such as walking is appropriate in Week 2. Avoid anything that significantly raises blood pressure, causes facial flushing, or puts you at risk of accidental impact to the face.
Weeks 3 to 6: Swelling Continues Resolving
The eyelids continue to refine through this period. Some patients notice that the upper eyelid incision line is slightly pink or has some residual firmness along its length. This is a normal phase of incision healing and does not indicate a problem; the incision gradually softens and fades over the following months.
“Patients sometimes come to me concerned that the upper eyelid crease looks a little uneven or thickened at three weeks,” says Dr. Dhruv Chavan, blepharoplasty surgeon at Dr. DC Plastic Surgery. “I explain that the incision line itself is still maturing. It takes 3 to 6 months for the scar to soften and for the crease to settle into its final position. Comparing the eyelids at three weeks to before surgery is not a useful comparison; the final position takes time.”
Activity progression in Weeks 3 to 6:
Moderate exercise can typically be resumed in Weeks 3 to 4 for most patients. Swimming should wait until the external incisions are fully healed and closed (confirm with your surgeon). Avoid intense exercise that significantly elevates heart rate and blood pressure before 4 to 6 weeks.
The Upper Eyelid Scar: What to Expect?
For upper blepharoplasty, the incision runs in the natural crease of the upper eyelid. When healed, this is one of the most naturally concealed incision positions in facial surgery: the crease itself hides the fine line, and even without makeup the scar is rarely visible to others.
The stages of upper eyelid incision healing are predictable:
Week 1 to 2: Incision line is pink, may have some visible suture marks. Weeks 2 to 6: Pink fades to red-pink, incision line is visible on close inspection but not obvious. Months 3 to 6: Incision softens, fades to skin tone, and settles into the crease. Month 12 onward: The crease appears natural, and the incision is typically invisible without deliberate close examination.
For the incision to heal well, keep it out of direct sun during the first 12 months. When outdoors, wear UV-protective sunglasses or apply a thin layer of SPF 30 or higher around the eye area once the incision is fully closed. Indian skin phototypes are at higher risk of post-inflammatory pigmentation in healing incisions, and sun protection is particularly important.
Lower Blepharoplasty Recovery: What Is Different?
For transconjunctival lower blepharoplasty (no external incision), there is no external scar to manage. Recovery is generally faster than transcutaneous lower blepharoplasty with less visible bruising, though swelling under the eyes takes several weeks to fully resolve. Patients with lower blepharoplasty plus fat repositioning may notice an irregular contour under the eye in the early weeks as the repositioned fat consolidates; this settles over 4 to 6 weeks.
For transcutaneous lower blepharoplasty (sub-lash incision), there is a fine incision just below the lower lash line. This scar heals very well for most patients due to the thinness of the lower lid skin, but the same sun protection and gentle handling principles apply.

How long before I can wear eye makeup after blepharoplasty?
Eye makeup can typically be resumed at 10 to 14 days for upper blepharoplasty, once the incision is confirmed healed at your post-operative review. Apply and remove all eye products gently during the first several weeks, avoiding any rubbing or significant pressure on the lid or around the incision. When can I drive after blepharoplasty?
Driving can resume once your vision is clear and comfortable and you are not taking any medication that affects alertness. For most patients, this is from around Day 7 to 10, after the post-operative review and assuming vision has normalised. Do not drive on the day of surgery or for the first several days while using any prescribed pain relief or while vision is blurred from residual ointment or swelling.
Is the result permanent?
Blepharoplasty produces lasting structural changes. The excess skin removed from the upper eyelid does not grow back. The fat addressed in lower blepharoplasty does not return in the same form. However, the ageing process continues after surgery, and over the years the eyelid and brow area continue to change with the rest of the face. What if my eyes feel dry after blepharoplasty?
Temporary dry eye is a common experience after blepharoplasty, particularly upper blepharoplasty, as the lid position adjusts and normal blink mechanics temporarily change. Preservative-free lubricating eye drops recommended by your surgeon are the appropriate management. The dryness typically resolves over several weeks. Patients with pre-existing dry eye syndrome should discuss this at consultation before surgery, as blepharoplasty requires additional pre-operative assessment for dry eye. If dryness is severe, worsening rather than improving, or accompanied by any significant corneal discomfort, contact your surgeon.
Can I combine blepharoplasty with other procedures and does that affect recovery?
Yes.
