Some patients come to a rhinoplasty consultation because they cannot breathe through their nose. Others come because they want to change the shape of it. Many come with both concerns at the same time. The two problems are related but they require different procedures, and sometimes they require both performed together. Understanding which procedure addresses which problem is the starting point for making the right decision.
Septoplasty corrects a deviated nasal septum to improve nasal breathing and produces no external change in appearance. Rhinoplasty reshapes the external structure of the nose for cosmetic or combined cosmetic and functional reasons. When both problems are present, septorhinoplasty combines both in one operative session. Dr. Dhruv Chavan at Dr. DC Plastic Surgery, Pune, is an MCh Plastic Surgery qualified plastic surgeon and APSI member performing septoplasty, rhinoplasty, and septorhinoplasty at six Pune clinic locations.
What Is Septoplasty?
The nasal septum is the wall of cartilage and bone that divides the nose into left and right passages. When this wall is straight, airflow through both nostrils is roughly equal. When the septum is deviated, it is displaced toward one side, reducing airflow through the narrower passage.
Septoplasty straightens and repositions the deviated septum through incisions made entirely inside the nasal cavity. There are no external incisions and no external change to the appearance of the nose. The nose looks the same before and after. The goal is functional: to restore adequate nasal airflow.
Conditions septoplasty may help:
- Nasal obstruction on one or both sides attributable to the septum position rather than other causes such as turbinate swelling, polyps, or allergy.
- Chronic mouth breathing or sleep disruption caused by structural nasal obstruction.
- Snoring related to structural nasal narrowing, where the airway restriction is caused by the septum.
- A crooked nose where the external deviation is driven by the septum pushing the tip or dorsum off-centre. In these cases, correcting the septum can also improve the external appearance to a degree.
What Is Rhinoplasty?
Rhinoplasty reshapes the external structure of the nose. It addresses the parts that are visible: the bridge, the tip, the nostrils, and the overall proportions of the nose in relation to the face.
Common reasons patients consider rhinoplasty:
- Dorsal hump: A bump on the bridge that the patient would like reduced or smoothed.
- Tip shape: A wide, bulbous, or drooping tip the patient would like refined.
- Nostril width: Wide nostrils that can be narrowed through alar base reduction.
- Crooked nose: A nose that deviates visibly to one side. Where this is caused by deviated nasal bones or asymmetric cartilages (not just the septum), rhinoplasty is required to address the external structure.
- Breathing combined with cosmetic concern: When the obstruction is caused by collapsed external nasal valves or weak alar cartilages rather than the septum, rhinoplasty addresses both functional and cosmetic goals simultaneously.
Rhinoplasty is an elective surgical procedure. It has risks, and results depend on anatomy, healing, and following aftercare instructions. It is not appropriate for everyone and the consultation with a qualified surgeon determines candidacy.
Septoplasty vs Rhinoplasty: The Key Differences
| Septoplasty | Rhinoplasty | |
| What it changes | Internal septum (no external change) | External shape of the nose |
| Purpose | Functional (breathing) | Cosmetic or combined |
| External splint | Not required | Required 7-10 days |
| Visible bruising | Less common | Expected, 1-2 weeks |
| Insurance coverage | May apply for documented functional indication | Not covered for cosmetic |
| Recovery to desk work | 5-7 days | 10-14 days |

What Is Septorhinoplasty and When Is It Recommended?
Septorhinoplasty performs both procedures in a single operative session under one anaesthetic. It is appropriate when a patient has both a documented functional problem with the septum and a cosmetic concern with the external nose they wish to address at the same time.
The practical advantages are meaningful: one anaesthetic, one recovery, one set of post-operative appointments. Two staged procedures mean two recoveries and two anaesthetics, which is both more difficult for the patient and more expensive overall.
There is also a surgical benefit. The cartilage trimmed or removed during septoplasty, which would otherwise be discarded, can be used as graft material in the rhinoplasty to refine the tip, support the dorsum, or correct structural deficiencies. This is a meaningful advantage of the combined approach.
“When a patient has both a deviated septum causing real breathing difficulty and a cosmetic concern they want addressed, I explain that combining the procedures in one session gives them the functional and cosmetic correction with a single recovery and allows me to use the septal cartilage as graft material in the rhinoplasty,” says Dr. Dhruv Chavan, plastic surgeon at Dr. DC Plastic Surgery, Pune. “The planning is more complex, but for the right patient, the outcome is better than staging the procedures separately.”

How the Decision Is Made at Consultation?
The correct procedure is determined by examining both the internal nasal anatomy and the external nasal structure. Internal examination covers the septum position, turbinate size, and internal valve integrity. External examination covers the dorsum, tip cartilage structure, nostril width, and overall nasal proportions.
“When a patient comes to me saying they cannot breathe and they also want to change the shape of the nose, I examine both problems separately and explain what is driving each one,” says Dr. Dhruv Chavan, rhinoplasty surgeon at Dr. DC Plastic Surgery. “Sometimes the crooked appearance is caused entirely by the septum pushing the external structure to one side, and correcting the septum alone straightens the external nose. Sometimes the cosmetic concern and the breathing problem are anatomically separate issues requiring different solutions. And sometimes both need to be addressed, in which case septorhinoplasty is the right approach. The examination tells us, not the patient’s guess before we look.”
This assessment requires training in both internal nasal anatomy and the external structural anatomy that rhinoplasty addresses. A plastic surgeon in Pune who performs both septoplasty and rhinoplasty has the appropriate qualification for the combined septorhinoplasty procedure.
Who May Not Be a Suitable Candidate?
Rhinoplasty and septorhinoplasty are elective procedures. Certain factors affect candidacy and are assessed at consultation:
Patients who smoke are at higher risk of healing complications and wound-healing problems. Patients with unstable medical conditions that increase anaesthetic risk require clearance from their treating physician before surgery can be scheduled. Patients who have unrealistic expectations about the degree of change possible given their anatomy, or who are seeking surgery to satisfy an external social pressure rather than their own considered decision, may benefit more from further discussion at consultation than from proceeding to a surgical plan. Patients who have had recent nasal surgery should allow adequate healing time before a subsequent procedure. For patients who are unhappy with a previous rhinoplasty, revision rhinoplasty is a separate, more complex category: see the revision rhinoplasty guide for more.
Preparation Before Septoplasty or Rhinoplasty
Dr. Dhruv Chavan provides a full pre-operative preparation list at the surgical planning appointment. General preparation includes:
Avoiding aspirin, ibuprofen, and blood-thinning supplements for two weeks before surgery, as these increase bleeding risk. Stopping smoking for at least four weeks before and four weeks after surgery, given the impact on wound healing. Pre-operative investigations (blood tests, ECG if indicated) are arranged based on the clinical assessment. The preparation instructions are tailored to each patient’s specific procedure and health status and are confirmed at the pre-operative appointment.
Risks and Considerations
Septoplasty, rhinoplasty, and septorhinoplasty are surgical procedures performed under anaesthesia. Like all surgery, they carry risks that are important to understand before making a decision.
Risks associated with these nasal procedures include: infection, bleeding, adverse reaction to anaesthesia, swelling and bruising during recovery, asymmetry in the healing result, numbness of the nasal tip (typically temporary but occasionally prolonged after rhinoplasty), need for a secondary or revision procedure, and in rhinoplasty specifically, the risk of over-correction or under-correction requiring further treatment.
Septoplasty-specific risks include: incomplete improvement in nasal airflow, and rarely, septal perforation (a hole in the septum), which is an uncommon complication.
Rhinoplasty-specific risks include: prolonged swelling particularly at the tip (which can take 12-18 months to fully resolve), skin irregularities, changes to nasal tip sensation, and visible scarring at the columella incision for open rhinoplasty.
Results depend on individual anatomy, skin type and thickness, healing, and following all aftercare instructions. Individual outcomes vary and no specific result can be guaranteed.
For information on the rhinoplasty procedure step by step, see the rhinoplasty procedure guide and the rhinoplasty recovery timeline.
Urgent-Care Guidance After Nasal Surgery
Contact the clinic or an emergency service immediately if you experience any of the following after septoplasty, rhinoplasty, or septorhinoplasty: fever above 38 degrees Celsius, spreading redness or warmth around the nose, severe or rapidly worsening swelling, fresh red bleeding that does not settle with gentle pressure, any difficulty breathing that feels acute and worsening, or chest pain. These symptoms should never be managed at home without clinical guidance.

Frequently Asked Questions
What is the difference between septoplasty and rhinoplasty?
Septoplasty corrects the position of the nasal septum (the internal wall between the two nasal passages) to restore nasal breathing. It produces no external change in appearance. Rhinoplasty reshapes the external structure of the nose: the bridge, tip, nostrils, or overall proportions. The two procedures can be performed together as septorhinoplasty when both functional and cosmetic correction are needed in the same patient.
Will septoplasty change how my nose looks?
Septoplasty does not change the external appearance of the nose. All incisions are inside the nasal cavity. If the crooked external appearance of the nose is caused primarily by the deviated septum pushing the external structure off-centre, some improvement in visible symmetry may result from septoplasty alone. However, for patients who want a specific change in shape, size, or tip definition, rhinoplasty is the appropriate procedure. This distinction is determined at clinical examination.
Can I have septoplasty and rhinoplasty at the same time?
Yes. Septorhinoplasty combines both procedures in a single operative session. This is generally more efficient than two separate procedures and allows the cartilage removed during septoplasty to be used as graft material in the rhinoplasty. Whether the combined approach is appropriate is determined at consultation after examination of both the internal and external anatomy and a full discussion of the patient’s goals and health status.
Is septoplasty covered by insurance in India?
Septoplasty performed for a documented functional indication, specifically nasal obstruction caused by a deviated septum affecting breathing and daily function, may be partially covered by some health insurance plans. Coverage and documentation requirements vary between insurers. Rhinoplasty for cosmetic reasons is not covered. At Dr. DC Plastic Surgery, the team can advise on documentation requirements when a functional indication is being explored with an insurer.
How long is recovery from septoplasty vs rhinoplasty?
Septoplasty alone typically allows return to desk work in 5 to 7 days. There is no external splint, and periorbital bruising is less common than with rhinoplasty. Rhinoplasty requires an external nasal splint for 7 to 10 days, with most patients socially comfortable from 10 to 14 days onward as bruising fades. Septorhinoplasty recovery follows the rhinoplasty timeline rather than septoplasty alone. For breathing-related considerations in nasal surgery, see can nose surgery improve your breathing.
