Non-surgical rhinoplasty uses injectable dermal filler to temporarily alter the shape of the nose without surgery. It is a useful procedure for a specific and limited set of concerns, and it carries risks that are more serious than most patients expect from an injectable treatment. Understanding what the procedure involves, what it is and is not capable of achieving, and what the significant risks are is essential before making any decision about nose filler in Pune.
Non-surgical rhinoplasty uses hyaluronic acid filler injected into the nose to smooth a dorsal hump, improve symmetry, or adjust tip projection. It is temporary (lasting 9 to 18 months), cannot reduce the size of the nose, and carries a small but serious risk of vascular occlusion. It is appropriate for a specific and limited group of patients. Dr. Dhruv Chavan at Dr. DC Plastic Surgery, Pune, is an MCh Plastic Surgery qualified plastic surgeon and APSI member assessing nose filler and rhinoplasty candidates at six Pune clinic locations.
What Non-Surgical Rhinoplasty Involves?
Non-surgical rhinoplasty involves injecting a small quantity of hyaluronic acid dermal filler into precise locations in and around the nose to create a visual change in nasal shape. The procedure is performed in a clinic setting, takes approximately 15 to 30 minutes, and involves minimal immediate downtime.
The filler does not reduce the nose. It adds volume. The visual effect of making a nose appear smaller or smoother comes from strategically placing volume to camouflage or balance the feature that is drawing attention. The most common example: a dorsal hump is not reduced by filler. Instead, filler placed above and below the hump raises the dorsum above the hump and smooths the bridge at the radix (the top of the nose), making the hump appear less prominent by removing the contrast between the straight bridge and the projection of the hump. The hump itself is unchanged.
What Non-Surgical Rhinoplasty Can and Cannot Do?
Understanding the boundary between what nose filler addresses and what it does not is the most important part of the decision for any patient considering this procedure.
What non-surgical rhinoplasty may address:
- Dorsal hump camouflage: Smoothing the appearance of a mild to moderate bump on the nasal bridge by filling above and below it, reducing the contrast.
- Radix augmentation: Adding height at the top of the nose (the radix) to improve the nasal profile.
- Mild tip projection: Adding a small amount of volume at the tip to improve projection in patients with a low or flat tip.
- Minor asymmetry: Correcting a slight unevenness in the nasal bridge by filling the depressed side.
- Post-rhinoplasty touch-up: Addressing a small irregularity after surgical rhinoplasty without returning to the operating room.
What non-surgical rhinoplasty cannot do:
- Reduce nasal size: Filler adds volume; it cannot make the nose smaller, reduce the width of the bridge or nostrils, or narrow a wide tip.
- Correct a significantly deviated nose: Structural deviation of the nasal bones or septum is a surgical problem.
- Improve nasal breathing: Filler has no effect on the nasal airway.
- Provide a permanent result: All hyaluronic acid filler dissolves over time. Results typically last 9 to 18 months, after which the procedure must be repeated to maintain the effect.
- Replace surgical rhinoplasty for significant structural concerns: Patients who need a meaningful change in nasal shape, size, or function require surgical rhinoplasty. Filler is an appropriate interim option for some, or a sufficient solution for a small group.
The Risks of Non-Surgical Rhinoplasty
The nasal region has a dense and complex blood supply with numerous small vessels that connect to the ophthalmic circulation. Injecting filler into or adjacent to these vessels can cause vascular occlusion: the vessel becomes blocked, either directly by filler or by pressure from surrounding filler. Vascular occlusion in the nose can cause skin necrosis (tissue death) at the tip, alar, or columella, and in the most serious cases, filler entering the ophthalmic circulation can cause permanent vision loss.
This is not a rare theoretical risk invented to discourage patients from the procedure. Vascular complications from nose filler have been reported in medical literature from clinics around the world. The nose is consistently identified as one of the highest-risk areas for filler injection among all facial regions.
The risk is minimised but not eliminated by: injection being performed by a qualified medical professional with anatomical training, use of hyaluronic acid filler (which can be dissolved with hyaluronidase if a complication is identified early), use of a blunt-tipped cannula rather than a sharp needle for some injection points, and injection performed in a facility prepared to manage the complication with hyaluronidase immediately.
It is not eliminated by the use of cannulas, slow injection technique, or any other technical precaution. A practitioner who offers nose filler without disclosing vascular occlusion risk has not adequately counselled the patient.
“I counsel every non-surgical rhinoplasty patient in the same way I counsel surgical patients,” says Dr. Dhruv Chavan, plastic surgeon at Dr. DC Plastic Surgery, Pune. “I explain what the filler can change, what it cannot change, and what the serious risks are. The vascular occlusion risk in the nose is small in absolute terms but the consequence of a missed or delayed vascular event is significant: skin necrosis or vision loss. Patients deserve to understand this before agreeing to the procedure. My view is that nose filler done by someone without the training to recognise and treat a vascular complication immediately is a procedure performed without the safety net the patient deserves.”
Other risks of non-surgical rhinoplasty include: bruising and swelling at the injection sites (typically resolving in 3 to 7 days), asymmetry, filler migration, delayed nodule formation, infection, and unsatisfactory result requiring dissolution or repeat injection.
Hyaluronidase: Why Reversibility Matters?
Hyaluronic acid filler can be dissolved using hyaluronidase, an enzyme injected at the filler site. This is the treatment for vascular occlusion when identified early: hyaluronidase is injected in sufficient quantity around the affected vessel to dissolve the filler and restore blood flow. It is also used electively to remove an unsatisfactory filler result.
The reversibility of hyaluronic acid filler is a meaningful safety advantage compared to permanent fillers. This is one of the reasons hyaluronic acid is the appropriate choice for nose filler, and non-hyaluronic acid permanent fillers should not be used in the nose given the increased risk of late complications and the absence of a reversal agent.

Who May Be Appropriate for Non-Surgical Rhinoplasty?
Non-surgical rhinoplasty is appropriate for a specific and limited group of patients: those with a mild to moderate dorsal hump who want to improve the profile without surgery, patients seeking radix augmentation or minor projection improvement without a permanent change, and patients who are not ready for surgery or wish to preview a change before committing to a surgical rhinoplasty. It is also appropriate for patients with small residual irregularities after surgical rhinoplasty.
“I see patients who come asking for nose filler when what they actually want is a smaller nose, a narrower bridge, or better breathing,” says Dr. Dhruv Chavan, rhinoplasty surgeon at Dr. DC Plastic Surgery. “For those patients, filler will not achieve their goal and telling them this clearly at the consultation is more helpful than performing a procedure that will not satisfy them. For the patient who has a clear bump on the profile and wants to see whether smoothing it changes their appearance, or who wants to improve their nasal profile without surgery while they think about their options, non-surgical rhinoplasty is a reasonable choice if they understand what it does and does not do.”
When Is Surgical Rhinoplasty the Better Choice?
Surgical rhinoplasty is the appropriate procedure for patients who want: a meaningful reduction in nasal size, narrowing of the bridge or nostrils, significant tip reshaping, correction of a structurally deviated nose, improvement in nasal breathing, or a permanent change. For these goals, filler is not an appropriate substitute. For a detailed overview of what surgical rhinoplasty involves and who it is for, see the rhinoplasty overview and the rhinoplasty consultation guide. Urgent-Care Guidance: Signs of Vascular Complication After Nose Filler
Contact the clinic or an emergency service immediately if you experience any of the following during or after non-surgical rhinoplasty: sudden sharp pain during injection beyond expected discomfort, skin that turns white (blanching) or develops a blue-grey or mottled appearance at or near the injection site, sudden change in vision, blurring, or loss of vision in either eye, headache beginning immediately during or after injection, or any skin area that becomes progressively dark or begins to show signs of breakdown. These signs suggest vascular occlusion and are a medical emergency. Do not wait for symptoms to resolve on their own. Time to treatment is critical in determining outcome.

Cost of Non-Surgical Rhinoplasty in Pune
| Item | Approximate Cost |
| Non-surgical rhinoplasty (hyaluronic acid filler) | Rs. 20,000 to Rs. 45,000 per session |
| Hyaluronidase dissolution (if required) | Rs. 5,000 to Rs. 15,000 |
Cost depends on the volume of filler required and the number of areas treated. The fee at Dr. DC Plastic Surgery covers the consultation, the procedure, and the standard follow-up. Hyaluronidase for dissolution is priced separately and used only when clinically indicated or requested by the patient.
The result of non-surgical rhinoplasty lasts 9 to 18 months. After that period, the filler dissolves and the nose returns to its original shape. Maintenance requires repeat sessions at a similar cost. Patients who then proceed to surgical rhinoplasty do not carry any additional risk from previous filler, provided the filler has been appropriately dissolved or has fully absorbed before surgery.

Frequently Asked Questions
Is non-surgical rhinoplasty safe?
Non-surgical rhinoplasty is a medical procedure with real risks, the most serious of which is vascular occlusion: filler entering or compressing a blood vessel in the nose and restricting blood flow to adjacent tissue or, in rare cases, reaching the ophthalmic circulation and affecting vision. These complications are uncommon but serious. The risk is reduced by choosing a qualified medical practitioner with anatomical knowledge, using hyaluronic acid filler (which can be dissolved), and having the procedure done in a facility prepared to treat a vascular complication immediately. No injection technique eliminates the risk entirely. Patients who understand this risk before proceeding make a more informed decision. The procedure is not appropriate for practitioners without the training to manage complications.
How long does nose filler last?
Hyaluronic acid nose filler typically lasts 9 to 18 months, after which it is naturally absorbed and the nose returns to its pre-treatment appearance. The duration varies depending on the specific product used, the volume injected, and individual differences in metabolism. Filler in the nose tends to last longer than in areas with more movement, such as the lips. Maintaining the result requires repeat sessions. There is no cumulative build-up of permanent filler with hyaluronic acid, as each session adds temporary material that will eventually dissolve.
Can I have nose filler before surgical rhinoplasty?
Yes. Some patients use non-surgical rhinoplasty as an interim step: to preview how a change in bridge or tip contour affects their appearance before committing to surgery. If a patient later decides to proceed with surgical rhinoplasty, the filler should ideally be fully absorbed or dissolved with hyaluronidase before surgery, as residual filler can complicate the surgical anatomy. This is discussed at the rhinoplasty consultation. For information on what rhinoplasty surgery involves, see the rhinoplasty aftercare guide and an overview of the candidate criteria for rhinoplasty.
What is the difference between non-surgical rhinoplasty and surgical rhinoplasty?
Non-surgical rhinoplasty uses temporary dermal filler to add volume and create visual changes in nasal contour. It is suitable only for a limited range of concerns (primarily hump camouflage and minor contour adjustments), the results last 9 to 18 months, and the nose returns to its original shape when the filler dissolves. Surgical rhinoplasty permanently reshapes the bone, cartilage, and soft tissue of the nose, addresses a much wider range of concerns including size reduction, structural correction, and breathing improvement, and provides a permanent result. The recovery for surgical rhinoplasty is longer, with most patients comfortable in public from 10 to 14 days and final results assessed at 12 to 18 months. For more on what age is appropriate for rhinoplasty, see the rhinoplasty age guide.
Who should not have non-surgical rhinoplasty?
Non-surgical rhinoplasty is not appropriate for patients who want to reduce the size of the nose, narrow the bridge or nostrils, correct a significantly deviated or crooked nose, improve nasal breathing, or achieve a permanent result. It is also not appropriate for patients with very thin nasal skin (where filler irregularities are more visible), patients with a history of previous filler complications in the nose, or patients who are not prepared for the possibility of needing hyaluronidase dissolution if the result is unsatisfactory. Patients who have had multiple previous rounds of nose filler should disclose this fully at consultation, as layered filler can create a less predictable anatomy.
